Information for Tax Preparers

Please make sure to submit the following documents for each client.

MY CHECKLIST OF DOCUMENTS

- W-2 (s)
- Federal 1040
- Declaration of estimated tax for 2007
- Federal Schedule C & 1099 (s) (If Self-Employed)
- Federal Schedule E & Tenant Listing (If you own rental)
- Your physical address of your employment (Your W-2 might state the corporate headquarters) The reason this is important is to verify that the correct tax has been withheld and that proper credit is being given. (Not all employers withhold correctly)
- Verification of when you moved into/out of Norwood Ex: Water/Gas/Electric, copy of Lease, Post office change of address form.
- Phone numbers for your client.
- Phone numbers for your office. (In case we have questions) Also, please print your name.
- Please also include the birthdates for your clients. Most instances all we see is a name and a social. Birthdates are important for determination of retires and individuals that are 16 to 18.
